A signed and completed green
card application form should be accompanied by certain supporting documentation.

- a valid passport or any other travel documents;
- two forward facing recent color photographs (size: 5 cm x 5 cm);
- proof of medical examination;
- birth certificate;
- marriage certificate, if applicable;
- divorce or death certificate, if applicable; and more depending on the
